Working primarily in the outpatient clinic, you’ll collaborate daily with physicians, nurses, social workers, administrators, and financial coordinators to deliver comprehensive, age-appropriate care during both wellness and illness.

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.

Key Responsibilities
60% CARE COORDINATION
Become the central hub of communication and organization for pulmonary hypertension patients as you:
- Coordinate care using established clinical protocols.
- Review diagnostic tests, labs, and clinical data to support decision-making.
- Ensure accurate documentation in progress notes, flow sheets, the Phoenix database, and the EHR.
- Collaborate with a multidisciplinary team including physicians, social workers, financial coordinators, and nursing staff.
- Manage triage calls and partner with physicians to assess and address patient care needs.
- Participate in program and committee meetings.
- Submit clinical information to insurance carriers for precertification/authorization.
- Provide cross-coverage to fellow coordinators as needed.
Help Patients And Families Better Understand Their Condition By
- Creating and maintaining educational resources for patients, families, staff nurses, and program teams.
Support Program Excellence By
- Collecting, reporting, and submitting required data to the Pulmonary Hypertension Association in alignment with accreditation standards.
Elevate Program Performance By
- Preparing quality metrics, attending protocol meetings, and supporting administrative initiatives.
- Participating in Quality Assurance/Performance Improvement activities to strengthen outcomes and operations.
Expand The Program’s Reach And Reputation By
- Participating in community outreach efforts and traveling to partner facilities as needed.
- Serving as a pulmonary hypertension content expert at local, regional, and national levels.
- 3 years of professional nurse experience; at least 1 year of previous transplant, outpatient or dialysis experience highly desirable.
- ADN required; BSN, bachelor's degree in nursing preferred
- Current IL RN license
- CPR (Cardiopulmonary Resuscitation)
- Preferred: Specialty certification in clinical or functional area of nursing granted by a national nursing organization
- Preferred: National Certification as Transplant Coordinator or other related specialty preferred.
Salary Range: $48.14 - 63.75 per hour
