As an LPN with Elara Caring, you’ll provide skilled nursing care under the direction of a Registered Nurse, ensuring patients receive safe, compassionate, and effective care in their homes.
You will:
Deliver skilled nursing care in accordance with the patient’s Plan of Care, physician orders, and established standards
Support patient goals through high-quality, individualized nursing visits
Communicate changes in patient condition to the RN Case Manager, physician, and care team
Promote patient safety and report unsafe conditions promptly
Educate patients and caregivers on self-care techniques
Assist with medication management and accurate documentation
Collaborate closely with the RN Case Manager for guidance and support
Help prevent avoidable hospitalizations and emergency room visits
Treat patients, families, and caregivers with respect, compassion, and professionalism
Graduate of an accredited nursing program (Certificate, Diploma, or Associate Degree)
Active State LPN/LVN license
At least 1 year of clinical nursing experience
Hospice or home health experience preferred
Ability to travel locally (approximately 50%)
Valid driver’s license, reliable transportation, and insurance
Ability to meet physical demands of the role, including lifting up to 50–100 lbs
