Position Description

Pride Health is hiring an RN – Case Manager to support our client’s medical facilityin West Allis, WI. This contract opportunity is a great way to start working with a top-tier healthcare organization!

  • Location: West Allis, WI – 53227 (within the service area; possible to float to neighboring areas)
  • Job Type: Contract
  • Duration: 13 weeks
  • Specialty: Home-Hospice Care
  • Schedule: Monday through Friday, 8 am – 5 pm
  • Pay Range for locals: $57.00 - $61.00 per hour
  • Pay Range for travelers: $2,980.00 - $3,120.00 per week

*Offered pay rate will be based on education, experience, and healthcare credentials.

Requirements:

  • 1 year of Home Health RN experience – required
  • 3 years of experience in clinical nursing – required
  • Active State of Wisconsin RN license or compact RN license – required
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) – required
  • Associate’s Degree in Nursing – required
  • A valid Driver’s License issued by the Division of Motor Vehicles – required

Responsibilities:

  • Manage a caseload of 20-30 patients per week, providing in-home care for individuals with multiple comorbidities such as CHF, COPD, cancer, dementia, and post-surgical needs.
  • Perform advanced nursing care, including wound care (wound vacs, pleural drains, ostomy care), IV therapy, central line management, and peripheral blood draws.
  • Utilize EPIC for documentation, operate Sapphire IV pumps, and coordinate with the support team (Manager, Lead, Scheduler) to ensure seamless patient care.
  • Travel to patient homes independently, requiring reliable transportation and adherence to professional dress code standards.
