Registered Nurse New

Pride Health is hiring a Case Manager Registered Nurse to support our client’s medical facility based in West Allis, Wisconsin. This contract opportunity is a great way to start working with a top-tier healthcare organization!

Responsibilities:

• Patient Care & Case Management – Manage a caseload of 20-30 patients per week, treating individuals with multiple comorbidities such as CHF, COPD, pneumonia, cancer, dementia, kidney disease, infections, and post-surgical needs.
• Medical Procedures & Treatment – Perform wound care (including wound vacs and pleural drains), ostomy care, peripheral blood draws, central line care, and IV administration using elastomeric and IV pumps.
• Charting & Documentation – Utilize EPIC/computer charting, ensuring accurate and timely documentation using a laptop and Sapphire IV pumps.
• Collaboration & Support – Work with a support team including a Manager/Supervisor, Lead, and Scheduler to coordinate patient care effectively.
• Professionalism & Requirements – Wear professional scrubs (any color allowed), provide own transportation for patient visits, and maintain active RN license, BLS (AHA), and at least 1 year of Home Health RN experience.

Qualifications:

• Experience: Minimum 1 year of Home Health RN experience
• Licensing & Certification: Must have an active state RN license and BLS certification (AHA).
• Clinical Skills: Proficiency in wound care (including wound vacs and pleural drains), ostomy care, peripheral blood draws, central line care, and IV administration using elastomeric and IV pumps.
• Technical Skills: Experience with EPIC/computer charting, laptops, and Sapphire IV pumps.
• Professional Requirements: Must provide own transportation for patient visits and wear .

*Offered pay rate will be based on education, experience, and healthcare credentials.
