Title: Licensed Practical Nurse (LPN)

Location: New Lisbon, WI

Job Type: 13 Weeks Contract

Shifts: PM Shift (2:00 PM - 10:00 PM), Rotating Weekends, Holiday Rotation

Pride Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. This position offers a 36-hour per week, 13-week contract. The ideal candidate will have experience in providing quality care to residents, administering medications, reporting changes in health status, and assisting with other nursing duties in a dynamic healthcare setting. Key Responsibilities:

• Administer medications to residents while following the 5 Rights.
• Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
• Communicate any change in resident condition to the charge nurse.
• Assist CNAs with answering call lights and other responsibilities as needed.
• Update physicians, family members, etc., regarding changes in condition.
• Ensure all procedures and responsibilities are followed accurately and effectively.
• Perform other duties as requested by the nursing team.

Qualifications:

• Must hold and maintain a Wisconsin LPN license.
• Current CPR/BLS certification required.
• 1+ years of related work experience as LPN.
• Exceptional accuracy and attention to detail required.
• Ability to work effectively in a fast-paced and dynamic healthcare environment.
